POSITION PURPOSE:
Ongwanada’s Service Planning and Care Coordination division plans, coordinates and facilitates the provision of quality services and supports, reviews the implementation and delivery of services and supports, takes corrective action as necessary, assists with problem solving, and advocates for the person served and their family. The Service Coordinator role requires ongoing monitoring and periodic reviews to adjust service plans as needed, ensuring that the support provided continues to align with each person’s evolving circumstances. With a focus on maintaining high standards of care, this position ensures that services are delivered in a timely and effective manner, with consistent follow-up to uphold the quality of care and supported individual satisfaction.
DUTIES AND RESPONSIBILITIES
- Manage the Integrated Service Planning process, ensuring compliance and operating as Chairperson for service planning meetings;
- Ensure planning processes remain Person Centered are detailed, reflective, responsive and choice driven for individuals, and families;
- Navigate Support Network through organization processes to ensure a coordinated delivery of supports and outcomes while responding to individual need;
- Ensure the individual and their family are an integral part of the process and that efforts are coordinated amongst all members of the support network;
- Following specific timelines for completion of annual plans and clinical reviews, ensuring Service Coordination meets QAM timelines and addresses needs of the Person Served
- Consult with internal and external supports to determine Support Networks and specific membership invites to meetings;
- Coordinate supports and services as required to meet identified goals, ensuring individual’s rights are upheld and wishes addressed;
- Provide information, support and resources to enable the individual to actively participate in decision making, self-advocacy and goal development;
- Develop Integrated Care Plan ensuring completion of all necessary documentation, including tracking documentation as required;

QUALIFICATIONS, SKILLS & KNOWLEDGE REQUIRED:
- Social Science degree or a diploma from a Social Science Program or related field of study;
- Minimum of three years of experience in a related field, preferably in a supervisory role;
- Experience supporting teams, individuals, families and knowledge of community supports;
- Experience & training in Person Directed Planning and an understanding of developmental disabilities;
- Ability to work independently and travel to a variety of locations;
- Valid Class G Driver’s license with full driving privileges, insurance and access to a vehicle;
- Successfully complete and maintain Non-Violent Crisis Interventions Training;
- Bilingualism (French) would be an asset.
