POSITION:

Responsible for the direct supervision of assigned Community Network of Specialized Care staff, the provision of third party oversight to assigned Outside Paid Resources (OPR’s) and for reporting on and achieving identified targets and outcomes. Responsible to adhere to the Services and Supports to Promote the Social Inclusion of Persons with Developmental Disabilities Act, 2008 including the Regulations on Quality Assurance Measures (QAM), other legal and mandated requirements, and policies and procedures of Ongwanada.

DUTIES AND RESPONSIBILITIES:

  • Engages external supports and resources, families, clients, other planning team members, in cooperation with CNSC staff to ensure appropriate planning and  supports are identified and implemented relative to case management requirements and achieving outcomes,;
  • Regularly meets with staff individually and as a department to review service targets, trends, service gaps and outcomes, and provide direction and feedback;
  • Ensures departmental activities adhere to service policies and legislation/regulation/policy directives/ guidelines;
  • Serves as a direct liaison between each assigned Outside Paid Resource or privately owned and operated day program (both of which are hereinafter referenced as Resource) and Ongwanada as per approved individual Agreements for Service;
  • Ensures through site visits and other means that the assigned Resource is adhering to needed Quality Assurance Measures (QAM) as per regulation specific to the individual(s) receiving service and support through the Resource;
  • Reviews Incident and Serious Occurrence information for individuals supported in the Resource, forwards the information to the appropriate Senior Manager within required timeframes, and ensures required follow up and documentation;
  • Performs other related duties as required.

QUALIFICATIONS, SKILLS & KNOWLEDGE REQUIRED:

  • Degree or diploma from a Social Science Program;
  • Excellent interpersonal, written and oral communication skills,
  • Ability to interact effectively and sensitively with clients, families, staff and other service providers
  • Experience working with individuals with a developmental disability/dual diagnosis and knowledge of local community resources would be an asset;
  • Knowledge and experience with Person Centered Planning, development of support plans and budgets and MCCSS Compliance/Quality Assurance Measures(QAM) regulations would be an asset;
  • Valid Class G driver’s license with full driving privileges and the use of a motor vehicle.
