POSITION:
Working throughout the Southeast Region, across sectors and communities, as part of the Transition Age Youth Program providing support coordination and monitoring to youth transitioning to adult developmental services, the Transition Age Youth (TAY) Coordinator will:
Engage with child welfare agencies across the South East Region to identify and plan for youth meeting eligibility criteria for developmental services;
Identify and engage individuals and services necessary for the development and coordination of a person- centered plan and budget to successfully transition from children’s services to adult developmental services;
Support youth through the transition process and monitor them through a period of adjustment to ensure a successful placement; serve as a resource to service agencies, the DSO, and to local planning tables.
DUTIES AND RESPONSIBILITIES:
- Provide short-term specialized support to transition-age youth who are leaving the child welfare system or who have other complex special needs while coordinating services and supports necessary to develop an appropriate plan for these individuals;
- Act as primary planning lead; working with existing supports including case manager;
- Facilitate planning, and coordination of services within and across sectors including clinical services and other services as appropriate;
- Plan, based on methodology where case outcomes can be measured against the plan objectives;
- Support person and their family at key transition points in and out of services, across continuum of services and across sectors.

QUALIFICATIONS, SKILLS & KNOWLEDGE REQUIRED:
- BSW or related education in social services; minimum of 5 years work experience in developmental services and or with transition aged youth;
- Demonstrated ability to network and develop plans for complex issues;
- Excellent interpersonal Skills;
- Knowledge of regional resources and service systems;
- Experience in/understanding of project management would be an asset;
- A valid Class G driver’s license, with full driving privileges and insurance is required, and access to a reliable vehicle, with travel throughout the province;
- Bilingualism (French/ English) would be an asset.
